The Community Pharmacy Scotland (CPS) Digital Pharmacy Platform enables pharmacists to deliver services that improve experiences and outcomes for patients with long term conditions (LTC's) in Scotland.

By giving visibility onto key pieces of data captured by the patient, pharmacists can improve the population health of Scotland through closer monitoring of patients with LTC's, making better informed decisions on their care and offering personalised support by communicating directly with the patient.

The CPS Platform enables pharmacists to effectively manage their patients, their conditions and pharmacy services. CPS can review data on community pharmacy services at a national level, including usage data on both the pharmacy and patient sides, service outcomes and feedback.

Community Pharmacy members who provide services on this platform can review the performance of their pharmacy's service, view any areas for improvement, high level quality indicators and evaluations of whether outcomes are being met.

By giving pharmacists access to key pieces of data generated by the patient, the pharmacist can make better informed decisions on the care of their patients and offer personalised support by communicating directly with them.
